Considerations for Problems and Unusual Findings in Laparoscopic Cholecystectomy

|
Laparoscopic cholecystectomy is the newest treatment modality to be introduced for the management of gall stone disease and is gaining rapid acceptance elsewhere. Despite the rapid and widespread acceptance, early data suggest that some
complications
and limitations are more common when compared to standard cholecystectomy.
The aim of this study was to identify several problems in laparoscopic cholecystectomy and reason of secondary laparotomy or operative conversion. The problems and unusual findings that encoutered in author's experience were grasping or
dissection
failure due to fibrotic and thickened wall, bleeding from cystic artery, limited visual field, gallbladder perforation and stone loss into peritoneal cavity, faint G.B. visualization during i.o.c. and cystic duct impacted stone.
The causes of 2ndary laparotomy were 2 cases of bleeding and 1 cases of bile peritonitis. Operative conversion was done in 3 cases, all of which revealed over 5 mm in wall thickness.
